One possible approach is:

Meet with her outside of the SRDS process.

Clearly outline to her the issues with her work and what your expectations are.

Discuss with her the reasons for this poor performance.

Explain that her performance must improve as her work is essential for the business or you may have to address issues more formally.

Offer her some support and training to assist her to improve her skills.

Put in writing what has been agreed along with a development plan and objectives.
